Please complete this form if your patient has a medical condition that is a
contraindication to COVID-19 vaccination. This form should be completed only if different methods of vaccinating against
COVID-19 have been considered and you have determined that your patient’s medical condition precludes your patient
from receiving any of the COVID-19 vaccines.
Please refer to the Center for Disease Control’s Interim Clinical Considerations for Use of COVID-19 Vaccines Currently
Authorized in the United States for determining whether a particular medical condition is contraindicated. As further
guidance, the following are NOT considered contraindications to COVID-19 vaccination:

• Local injection site reactions after (days to weeks) previous COVID-19 vaccines (erythema, induration, pruritus, pain, etc.)
• Expected systemic vaccine side effects experienced from previous COVID-19 vaccines (fever, chills, fatigue, headache,
lymphadenopathy, vomiting, diarrhea, myalgia, arthralgia)
• Vasovagal reaction after receiving a dose of any vaccination
• Being an immunocompromised individual or receiving immunosuppressive medications
• Autoimmune conditions, including Guillain-Barre Syndrome
• Allergic reactions to anything not contained in the COVID-19 vaccines, including injectable therapies, food, pets,
venom, environmental allergens, oral medication, latex, etc.
• Breastfeeding
• Pregnancy
• Immunosuppressed person in the employee’s household
• Alpha-gal Syndrome
• History of Bell’s palsy
• The COVID vaccines do not contain egg or gelatin; allergies to these substances are not contraindications

I recently accepted a contract position with Aya Healthcare, a nurse travel agency. After completing my employment drug screen, physical, Tspot (tuberculosis screen) I submitted my medical documents pertaining to my vaccine records as I had done with my prior contracts with them back in 2020. I also completed a vaccination Titer back in 2020. My PCP has already sent a medical vaccination declination letter pertaining to my medical evaluation. Aya Healthcare has refused to accept my medical records as legitimate and they have sent me an email stating they require my medical documents to come from the facilities that gave me my vaccinations at birth which is completely illegal. This is a California based corporation and is the prime example of the negations of lawful free enterprise where individuals who are employees of Aya Healthcare hide behind their corporate email accounts and make fraudulent embellishments about their business authority requirements without being held accountable for sequestering US Citizens from gainful employment do to their political liberal fascist party agenda to scam and sequester the public to perpetuate control. They have also ignored my inquiries pertaining to they're concentrated focus about my private healthcare information and how it has become a point of contention, understanding that it is absolutely illegal to disregard medical documentation. They have also sent me messages stating that the contracted company is also involved in the affair and they speak for the contracted company as well which is also illegal. They also "require" (and everything is a requirement with this company and always last minute with no reason except with the notion jump through our hoops or we will stop your progress) that I do an additional Titer that was already performed in 2020. They refused to acknowledge my rights and continue to send me messages about coordinating with their legal department when I exercise my rights under constitutional law. The employees consistently use a language of "require" and "recommend" to prolong closure of legal matters and refuse to uphold the autonomy of the American Civil Liberties.
